The Italian beach etiquette says that “fatty, ugly and unpretty women are not allowed to appear on the beach topless.”

The "beach etiquette"... Not a law, not any legal requirement, the "beach etiquette".

All this say is some peoples don't like seeing some type of woman topless and these woman are frown upon if they dare go topless. It's like if you're at a restaurant and you're burping at every bite you take: you won't be thrown in jail for doing so but, you'll likely be remembered one way or another that burping in public isn't a good idea. Yet, in some cultures, burping after a meal is considered polite!

not in many places in Italy...

They may just give dirty looks and make comments. That's all the enforcement they need. People really pride themselves more on their appearance in general in Italy. It's cultural difference... I've noticed that young Italians also seem to be generally thiner, or let's say less apt to be obese, so it probably doesn't affect the vast majority of people who would prefer to be topless... That's just how it is over there in many places. If you want to see "fatty's" on the beach, you don't have to go so far!

I have read several sources that claim that in the Palermo region of Italy, there is a nude beach, where everyone is supposed to be nude. On the other beaches, most people wear clothing, but women are not required to, only men are required to. Makes sense to me.
