When I first started seeing SPs, I had an ingrown hair in my crotchular region that I convinced myself was a herpes lesion. So I went to get tested. This was back in the day when they still did the urethra swab, which is a gentle way of saying they take a wooden stick to scrub the inside walls of your dick.

In between the pain and guilt, I promised myself that if it came back negative I'm done with SPs. It was negative, but the thought of beautiful women I can sleep with for relatively little effort made me break my promise fairly quickly lol.

Fast forward some decades later, still been fortunate to have never caught anything (not even the ones that seem easier to catch with unprotected oral like chlamydia and herpes). True, but it's also easier to be paranoid because the condom only covers your member. Your whole crotch interacts with the woman, not just your wang. So it could still be something to worry about even if the lesion of concern isn't directly on your genital.

I was personally more scared of herpes II (the oral one is carried by nearly all adults, even if one has never had a mouth cold sore) than HIV because I was led to believe you could still catch it with condom use. But my luck with being a man ho can't be so immaculate that I've never met someone with herpes II (not everyone knows they have it). Even though I've never been with someone with blisters in their privates, spreading herpes II must not be as easy as I imagined because I am still testing negative for antibodies.
