From my experience, gfe has become the local industry norm for agencys and independants for quite some time.
In the present market, safe services are in extremely low demand and gfe is offered by anyone and everyone.
By definition gfe really boils down to an sp that kisses (pecks, lfk, or dfk they are debatable because dfk seems to be shifting into a pse service slowly but surely) , and a bbbj with daty, digits, and maybe a tongue in her butt if you're lucky.
As for gfe+ or gfe++ they are probably just a play on words and symbols by agencys.

I asked a single booker once, so I don't assert that his definition is universally accepted. "GFE+ means that CIM is an available with extra $. WS may be also."
As far as I'm concerned GFE includes BBBJ and DFK anything else, CIM and Anal is extra but to start with ++ or +++ or +++++ doesn't make sense.
I know of one agency that clearly advertises its service as GFE+ - WildTimes. There, GFE+ means CIM included. (It's been a while since I've called Jimmy though, so not sure if I'm outdated.)
I've generally taken the '+' to mean CIM included. But there's no set standard around that definition. I don't think Greek is part of the '+', Greek veers into PSE, for me at least.

GFE = LFK/DFK, DATY, MPOS, MSOG, BBBJ.
GFE+ = All the above plus CIM
GFE++ = All the above plus GREEK
The last one generally falls into PSE category which may or may not include FACIAL).

There are a ton more activities that are mostly considered PSE or fetish/not typically included in the general GFE definition that I enjoy and will engage in with my companion of the moment, therefore the (+) part of the GFE.
I think the added (+) or (++) of GFE is very personal to each lady and shouldn't fall into a set definition.
