First of all, whether being first or not, I always ask the ladies to get a shower + using mouthwash when they come to my place ( Yes, I do outcall only). Not a single lady has ever refused this request as I do it very subtly. So hygiene has never been an issue for me regarding this subject. Its all about my mentality for the very reason that I stated above.
