Most hotels don't charge extra for the second person, but some low end places do. The low end places, however, might just let you pay cash and use an assumed name.
I personally won't give my last name. If they insist, call another agency. I have never had a problem giving just my first name and room number, and in the few cases they asked for more, I refused, and they did business with me anyway.
