Then there's Asservissante. Used them 6 years now, maybe for 15 sessions, and I think they were late once, by about 10 minutes. And that's it. Not bad. If Asservissante can do it, why can't the others?

Yes, I agree whole-heartedly.
Martin@Asservissante is the best in biz managing customer relationship.
After so many years and so many agencies comes and go, Martin@ Asservissante remains in the business, despite no longer on the top spot. That tells you something about his loyal customer base. You can always count on Martin keeping you informed if it is going to be late, even for 10-15 minutes. One of his policy got criticized recently was getting the girl out exactly on time. He also puts at 15-30 minutes between calls. This is in the expense of letting the girl waiting a few minutes in the car before each apt.
Because of this best practice, sometime he will get the short-end of the stick. Say a bad client cancels an apt in the last minute, his girl may not get a call in that time slot. It is too late to find another client to fill that slot without delaying another appointment.
So please do not cancel appointment at the last-minute with him. I did that once a few years back and he was furious. He said I did not want customers like you. And I really regretted I did that to him. We made up later. So be fair to him.

Devilish has the worst practice in the business. I do agree that we need to do something about it.
One of the major reason of the delay is a bad practice called over-booking, especially for a popular one. The other side of the evil practice is some hobbyst would book back-up girl in case he did not get who he wants, and cancels her when he does.
Here is an example of the bad practice I encountered last year. I was to promise the girl (say Kim) would be available at the time you requested, say 12:00 am. The booker (say Mike) knew for sure Kim would not make that because she had a call started at 11:45 pm or even 12:00 am. Mike would make that promise regardless. When you called, Mike would say be there in 15 minutes. 15 turned into 30 minutes. Then when he felt like you got inpatient and feared that you would cancel the call, he would start pushing other girls that were available . I insisted on Kim and told him I would wait till 3 am if I had to. When she arrived, it was 1:15 am. See, I would rather he be honest with me up front, and just let me know she would be available around 1 am. It would have made the experience so much better. That is just my perspective.
Unless I have no other choice, I will not deal with them anymore. Too much hassle for me!

I got sort of the same deal with Kim as after 30 minutes Mike suggested an other girl at a discount. I had booked 3 hours with Kim. He said that Kim didn't show up for work on that night. Could be true, I was her first appointment and they gave me a choice to refuse the girl or keep her for less time if I wanted. So they are not all bad either.
It actually turned out real good as the other girl was great and I saw her again and again.
Remember that smaller agencies don't have to deal with so many girls, the more girls you have the more problems you have.
Drivers availability is also a consideration, Usually each girl is assigned a driver for the night. That driver is assigned so many girls. If the appointments are not in the same location, you will get delays too because of the traveling time to service all the girls the driver has to deal with.

LBT, I am glad that worked out for you. Agency usually does not cancel a 3-hr apt for any client unless for an valid reason. It is possible Kim was indeed unavailable at the last minute. Did he offer any explaination?
In your senario, Mike should have let you know the first time you called that Kim was not confirmed. Instead he promised you an inventory he did not have. This is another bad parctice to get you hooked. At the last minute he sold you on other girls. You got lucky and it worked out for you.
In US, this practice is not uncommon in used car dealership. I hate this kind of business practice.
